Federal health inspectors from the Centers for Disease Control and Prevention inspect cruise ships twice a year, checking everything from kitchen equipment to dishwasher temperatures, and grade ships based on what they find. A score below 86 is considered failing. Cruise ships are also required to report all illness outbreaks affecting more than 3% of passengers or crew to the CDC. Inspections and outbreaks from 2010 to the present are shown below.

Medical-24-Hour Report Submission

No 24-hour report was submitted prior to arriving to Los Angeles on 29 January 2014 from a foreign port. The electronic acute gastroenteritis (AGE) database was pre-programmed to automatically submit a 24-hour report prior to entering a US port from a foreign port, but the Los Angeles (LAX) port code was listed in the system as not in the US (NUS) so no report was submitted. When medical staff realized an automatic report was not submitted they attempted to submit a manual 4-hour report through the electronic AGE database but could not override the system to change LAX to a US port. The 4-hour report attempt was returned electronically with a system error message stating NUS port codes cannot be selected when submitting a Maritime Illness and Death Reporting System (MIDRS) report. No documentation was available to show the report was submitted using an alternative method such as fax, email, or telephone.

Medical-Crew Member Late Reporting

A crew member who experienced 8 episodes of diarrhea with an onset at 0300 on 24 January did not report to medical until 25 January at 17:20. After examination he was deemed a reportable case. This employee was a musician who worked between the time of illness onset and reporting to the medical center. This employee received a written warming from his supervisor.

Medical-Close Contacts

The close contacts for a crew member deemed a reportable case on 25 February listed his cabin mate. Staff stated he did not have a significant other but this was not documented.

Medical-Acute Gastronenteritis (AGE) Log

A passenger reported to medical on 7 February with two episodes of diarrhea but was determined non-reportable because of intolerance of certain food items that she consumed the day before. This explanation was in the passenger's record but the underlying illness column on the AGE stated 'None.'

The final rinse pressure of the pot wash machine was 30 psi during the final rinse cycle. The attached data plate indicated the final rinse pressure should be between 15-25 psi.

The person in charge of this machine could not explain to their staff how to operate it. For example, they told the inspector there were different cycles for washing and for obtaining the proper final rinse temperature. When the inspector read the instruction manual for the machine, it indicated that all of the cycles included washing and a final rinse cycle, but each cycle was for a different length of time. The knob on the machine was not labeled and no one working in the area could determine which cycle the machine had to be on in order to check the final rinse temperature. Once a cycle was selected by staff, the inspector was able to determine the machine was meeting the proper wash and rinse temperatures.

Medical-Microbiologic Monitoring

The ship randomly collected potable water samples to analyze for the presence of E. coli in their distribution system on a weekly basis and these samples were analyzed in the medical center. The record for the samples collected on 8 November 2013 did not document any results but was signed by the doctor as an accurate and complete document. Staff stated they collected the samples as stated in the record but did not document the results. More than the minimum requirement of four random samples were collected and analyzed in November.

Pantry-Captain's Pantry

A washer and dryer combination machine for clothes was stored in the pantry. The machine was not installed or operational. There was food stored in the pantry.

The backflow prevention device installed on the hot water line for the filling of the whirlpool compensation tank was continuously leaking.

The RWFs had no hair and lint strainers and the pre-pump strainers were not cleaned, rinsed, and disinfected weekly. The two whirlpools had a combined system and the hair and lint strainer housing was empty. Staff stated that the strainer was removed around summer 2012 for an emergency maintenance and was not put back. There were strainers before the pumps for the whirlpool, pool, and spa pool systems, but there were no records documenting that they were cleaned, rinsed, and disinfected weekly.

Recreational Water Facilities-Fecal and Vomit Accident Response Plan

The fecal and vomit accident response plan did not meet the recommended pH levels of 7.2 - 7.5 to be used during disinfection per Annex 13.8 of the 2011 VSP Operations Manual and the clarifications document on the VSP web site. The plan stated pH levels of 7.0 - 7.8.

Additionally, some of the large plastic pallets in the beverage storage room had closed bottoms that made cleaning under the pallets difficult.

Recreational Water Facilities-

On 3 March, the starboard whirlpool spa was open all day and the halogen analyzer-chart recorder charts indicated the chlorine levels were less than 3 ppm and the pH was 8.0 or above. On 8 March, the starboard whirlpool spa was open from 01:02 - 08:09 and the values recorded on the halogen analyzer-chart recorder chart were 2.0 ppm, however the manual readings stated 4 ppm. Also, on 9 March, the chlorine residual levels were approximately 2.0 ppm and less than 3.0 ppm from 02:30 - 13:30 p.m. for the starboard whirlpool spa. This spa was never closed and there was no clarifications of discrepancies for these levels.

The light intensity was less than 110 lux in the area near the evaporator. Boxes of liquor were stored in this area. The company's Public Health Director stated the ship had been approved for an extension until 30 April for lighting upgrades.

A food worker had an onset of AGE symptoms on 9 March at 09:00, but did not report to the medical center until 10 March at 08:56. According to their time card, this crew member worked on 9 March from 06:58 - 11:37 and on 10 March from 05:30 - 06:58.

Medical-Crew Member Close Contacts

There were no close contacts documented for a food worker that had an onset of AGE symptoms on 9 March. According to the staff, this crew member had a close contact, but technical problems prevented them from entering the information into the STARDOCS medical surveillance system. A crew member with an onset of AGE on 15 February had a passenger listed as their immediate contact, but upon discussion with the medical staff, their immediate contact was actually a crew member. The interview for this close contact was documented in the food worker's medical paperwork.

Medical-Reportable Diarrhea Definition

A food worker had one diarrhea episode within 24 hours, stomach cramps, and muscle aches, but was listed on the AGE log as nonreportable. The medical staff were unaware of the new diarrhea definition in the 2011 VSP Operations Manual. According to the documentation, this individual was isolated for 48 hours as a precaution. Another food worker had an onset of AGE symptoms on 13 February. Their last symptoms were on 15 February at 09:00. Their symptoms included one diarrhea episode within 24 hours and stomach cramps, but they were listed on the AGE log as nonreportable. The underlying illness on the AGE log was recorded as 'nil of note'. According to the doctor, this crew member was thought to have faked their illness, but this was not documented. The crew member was only isolated for 24 hours and according to their time card, they worked several hours on 16 and 17 February. A nonfood worker had an onset of AGE symptoms on 15 February at 02:00, including two diarrhea episodes within 24 hours and stomach cramps. The underlying illness on the AGE surveillance log stated 'nil of note'. According to staff, this crew member's symptoms were thought to be caused by antibiotics, but this was not documented. This crew member had left the ship and their work record was not available.

Medical-AGE Surveillance Log

On 10 March, a passenger with four diarrhea episodes within 24 hours was recorded as nonreportable on the AGE log. Diabetes Type II was recorded as the underlying illness on the AGE log, but upon further discussion with medical staff, diverticulitis was the true cause of the diarrhea and why this case was not reportable.

Potable Water-Striping

The distillate line from the evaporators was not striped blue/gray/blue to the production chlorine injection point.

Potable Water-Cross-Connection Control Log

The specific plumbing components that contained backflow prevention devices were not mentioned in several areas of the cross-connection control log, including the deck 8 starboard hot and cold water lines, the deck 7 spa room water hose, the air gap for the technical water backflow on deck 1, and the air gap for the mineralizer. It was difficult getting information from staff on these devices due to the limited information.

Potable Water-Far Point Calibration

The far point halogen analyzer-chart recorder charts were not initialed and the calibration verification was not documented on the charts from 15 February - 3 March or on 6 March.

Recreational Water Facilities-Shock Halogenation

There was no shock halogenation documented on 10 March for the port whirlpool spa. Also, for this whirlpool, the same values of 4 ppm and 5 ppm for the start of shocking and 10 ppm at the end of shocking were recorded and repeated every single day with no variation. It could not be verified that shocking occurred on these days. There was no shock halogenation documented on the starboard whirlpool spa on 18 February - 5 March or on 10 March. Also, from 6 March - 9 March and on 11 March, the shock halogenation was documented as having a starting value of 4.0 ppm and an ending value of 10.0 ppm every day with no variation. It could not be verified that shock halogenation occurred at all during these times.

Recreational Water Facilities-

The starboard whirlpool spa was open from 14:00 on 1 March to 05:00 on 2 March and there was no calibration recorded on the halogen analyzer-chart recorder chart. Also, on all of the charts, the previous technician had used a 0-5 ppm scale manually written on top of the 0-10 ppm scale included on the halogen chart. There was no documentation and it was very difficult to determine the actual readings for residual halogen and pH.

Recreational Water Facilities-Residual Halogen Levels

During the inspection on the pool and whirlpool spa equipment rooms, it was discovered the dosing pumps for three RWFs were not working. The analyzer for the swimming pool was at less than 1.0 ppm, the starboard whirlpool spa was less than 1.0 ppm, and the port whirlpool spa was less than 2.0 ppm. The inspector verified the analyzer reading of the starboard whirlpool spa measured 0.24 ppm and that the low residual chlorine levels went off, but the staff in the ECR did not notify anyone or take any action upon hearing the alarms. All of these facilities were open. The inspector tested these facilities and measured the following readings: 0.29 ppm for the swimming pool, 0.55 ppm for the starboard whirlpool spa, and several minutes later the port whirlpool spa was measured at 1.54 ppm. Staff immediately closed all three facilities.

The person in charge of documenting and maintaining the RWFs was not knowledgeable enough about these systems to ensure they were operated correctly. For example, the shock halogenation for the port and starboard whirlpool spas was either not recorded or was shocked at the incorrect level for at least the past several weeks. There were several days when the manual logs stated that shock halogenation was taking place and the whirlpools were open at the same time with much lower halogen residual readings than what would be required for shocking. Also, the manual logs were filled out incorrectly. This individual did not properly record when the RWFs were open or closed and still recorded readings for the RWFs when the pools were closed, but wrote they were open on the manual logs. Staff were using multiple scales to record the chlorine residual level on the halogen analyzer charts and there was no documentation to determine which scale was correct. There were several days when the manual logs did not match the residual halogen or the pH levels on the halogen analyzer charts for the exact same period of time and there was no documentation why this occurred. The RWFs were not closed when the residual halogen levels were low as documented on several charts.

Personnel Casualties : 1 injury

On February 12, 2014, at approximately 0845 local time, the foreign-flagged cruise ship AZAMARA QUEST was underway in Monterey Bay, California, when a U. S. citizen passenger was struck by a wind-blown door that she was trying to close. The passenger was knocked down to the deck by the door. She sustained a fractured hip as a result of being knocked down. A determination was made by the Sector San Francisco Investigations Division Chief that there would be little or no value gained from conducting an informal investigation for this casualty. As such, this case was downgraded to the Data Collection level of investigation.

Fire or explosion

On 30 March 2012, late in the night, the passenger ship AZAMARA QUEST, with more than 1,000 people (590 passengers and 411 crew members) aboard was stranded off the southern Philippines, after a fire broke out in one of its engine rooms. The ship restored propulsion the following night and safely reached the harbour of Sandakan city in Malaysia's eastern state of Sabah on Borneo Island on Sunday 1 April 2012. One crew member was seriously injured and 5 suffered smoke inhalation.
