Federal health inspectors from the Centers for Disease Control and Prevention inspect cruise ships twice a year, checking everything from kitchen equipment to dishwasher temperatures, and grade ships based on what they find. A score below 86 is considered failing. Cruise ships are also required to report all illness outbreaks affecting more than 3% of passengers or crew to the CDC. Inspections and outbreaks from 2010 to the present are shown below.

Medical-Crew Illness Reporting and Monitoring

The crew of the Carnival Sunshine are to be commended for their prompt reporting of gastrointestinal illness symptoms. For the records examined during this inspection, exceptional promptness of reporting these symptoms to the ship's medical center was noted. The ship's medical staff had very good compliance with USPHS / VSP recommendations for monitoring these illnesses and isolation times based on crew position and on cabin mates and other close contacts.

Potable Water-Micriobiologic Monitoring

Record shows that the monthly representative sample locations need to be improved. Locations on the same deck were sampled in one month, and the same location was sampled in successive months. A suggested sampling plan for moving these sampling points around the ship was shared with the technical staff.

Recreational Water Facilities-Waterworks

Sediment was observed in all three of the water slides after they had been cleaned. A dust-like film was noted on two of them.

Gray and green mold was noted on evaporative coil housing . The condensate pan in this unit was clean.

Galley-Deck 9 - Potwash

The final rinse temperature of the in-use potwasher was 150-152°F at the pot surface and 180°F on the potwasher gauge. This temperature difference suggested that the gauge was not working properly.

The coupling for the backflow preventer to the juice machine had a leak when the line was moved.

The bain marie had an excess of steam causing it to condense on the shelf above the food and drip back into the open food on display.

Medical-Records of Isolation of Symptomatic Food Employee

Records documented that a food employee was isolated for only 10 hours and 30 minutes instead of the required 48 hours. The food employee reported to the medical center with AGE symptoms on 13 October at 10:08. Follow-ups on 14 October at 9:31 and on15 October at 9:33 found the employee was still having symptoms. However, the food employee was released from isolation on15 October at 20:00. Staff stated that the last symptom was actually at the time of presentation on 13 October at 10:08, but more symptoms were entered by mistake in the electronic medical record system.

Medical-Acute Gastroenteritis (AGE) Surveillance Log

A nonfood employee reported on 25 September having diarrhea, stomach cramps, no underlying illness, was dispensed antidiarrheal medication and was deemed a non-reportable case. There were two conflicting medical notes on his file, one saying that the symptoms were not above normal and another note saying that the symptoms were not normal for the individual.

Potable Water-Reagent for Water Test Kit

Staff was using a chlorine reagent that was not meant to be used with the test kit used by the ship. Staff was using this incorrect reagent/test kit combination (Scientific brand reagent with Palintest Pooltest 6 test kit) for measuring the free chlorine levels in the bunker chlorination station and at the far point.

Potable Water-Far Point

The free residual halogen measured by the halogen analyzer was not ± 0.2 mg/L (ppm) of the free residual halogen measured by the manual tests performed by the staff and the inspector. On a first check, the analyzer measured 1.23 ppm while the staff measured 1.15 ppm and the inspector measured 2.33 ppm. On a second check, the analyzer measured 1.10 ppm while the inspector measured 2.15 ppm. On a third check, the analyzer measured 1.04 ppm while the staff measured 1.87 ppm. The test kits of the staff and the inspector were the same model, but the staff was using a chlorine reagent that was not meant to be used with the test kit.

Recreational Water Facilities-Serenity Pool

Both antientrapment drain covers were loose, with missing screws, and with a 1 cm separation from the pool's floor. One of the drain covers had a cardboard stopper stuck to one of the drain edges to prevent it from moving. The pool was not open. This was corrected.

The bulkhead/flat grill frame profile strip was loose creating a difficult to clean space. The profile gaps continued along the right side of the flat grill.

A long black hair was noted on the technical space splash shield below the juice concentrate canister in the juice dispenser. The machine was taken out of service and cleaned.

Room Service-Deck 6 - Beverage Station

The juice dispenser water supply line backflow preventer was leaking from the atmospheric vent indicating a possible failure.

Galley-Deck 3 - Decks

The grout between the tiles in the hot galley and in walk in cooler #33 was rough, sometimes missing, and in poor repair, creating difficult to clean surfaces.

A mop was stored in a mop bucket in the forward galley corridor rather than hung up to dry.

Extensive leaks were noted from the deckhead seams above and near the entry area from the main passageway. These leaks also extended into the salad prep area and fell on an empty counter. No food or equipment was impacted.

The final rinse temperature did not reach a surface temperature of 160°F on 3 successive trials on a warewasher that had been used just prior to the inspection team entering the area. The thermo label and ship's maximum registering thermometer showed a maximum temperature of 150°F. This was corrected during the inspection.

The deckhead was leaking water from an unknown source from seams around the outside of the potwash canopy.

The final rinse temperature measurement was repeated three times with the same result. The potwasher was repaired while the team inspection team was aboard.

Recreational Water Facilities-Whirlpool Superhalogenation Records

According to the analyzer-chart records, proper superhalogenation was not achieved or not conducted on various dates for all the whirlpools. The superhalogenation did not reach 10 ppm on 3/16-4/20 and 4/22-4/26 for the Whirlpool Starboard. The superhalogenation was conducted for less than 1 hour on 12/15, 12/20, 12/31. 1/1, 1/2, and 1/3 for the Whirlpool Starboard. The superhalogenation was conducted for less than 1 hour on 12/26, 1/1, 1/2, 1/4, and 1/5 for the Whirlpool Port. Documentation was not provided for superhalogenation on 12/22, 12/31, and 1/3 for the Whirlpool Port. The superhalogenation was conducted for less than 1 hour on 3/16-18 for the Serenity Whirlpool. Documentation was not available for superhalogenation on 3/9 for the Serenity Whirlpool.

Water from the chilled water line attached to the deckhead in front of the dishwash machine control panel was dripping to the deck. In addition, water was dripping to the deck from the edge of the hood at the clean end of the machine and the adjacent technical hatch. Condensate had accumulated on the deckhead at the soiled end of the machine, but was not dripping.

The temperature of the final sanitizing rinse for the conveyor warewash machine was less than 160°F at dish-level. The temperature was measured using a long rod and a maximum registering thermometer. The temperature was approximately 155°F. The curtains remained in the machine during this first measurement, and the machine in operation prior to the temperature measurement. The final sanitizing rinse temperature was measured several more times using various methods, including a maximum registering thermometer on a long rod without the curtains, a maximum registering thermometer placed in the machine prior to the final sanitizing rinse arm, a 150/160/170°F color-changing thermolabel on a stainless steel holder placed in the machine prior to the final sanitizing rinse arm, and a 180°F color-changing thermolabel placed directly on the final sanitizing rinse arm. The temperatures were between 140 and 152°F and the color of the color-changing thermolabels did not change. This was corrected.

The plates, stored on the top shelf, above the counter were not covered or inverted. This was corrected.

Galley-Outside Appetizer Pantry

The cap was missing on the atmospheric vacuum breaker for the Apex Power solution. This was located across from the appetizer pantry.

Water was dripping from the deckhead speaker cover to the counter below. The counter was located next to the walk-in refrigerator. The area was not in operation at the time of inspection and no clean items or food were impacted. The area was immediately blocked off.

The deck behind the stack ovens was soiled with more than one day's accumulation of food debris. This was immediately corrected.

The potwash machine had been out of service for approximately 10 to15 days. According to the staff, the replacement part was expected to arrive the day of the inspection. A large amount of soiled pots and pans were observed in the potwash area.

A leak from the bottom of the undercounter dishwash machine was noted. Water had accumulated and pooled on the deck under the machine, around the perimeter of the room to the left of the machine, and under the undercounter refrigerators.

The door from the service area to the pantry was pinned open, allowing access for flying pests. The door was closed immediately. This was written on the previous inspection. The inspector returned to the area over one hour later and the door was pinned open again.

There were several food items, including 35 gallon jugs of vegetable oil that were stored up against and across from two flammable lockers inside of the walk-in refrigerator that contained paint, rust inhibitor spray, hair spray, barbicide, nail polish remover, isopropyl alcohol, nail polish, hair coloring, and other spa related chemicals.

Medical-Acute Gastroenteritis (AGE) Definition

In four cases, the medical staff had recorded information that individuals had symptoms that met the AGE definition in the 2011 VSP Operations Manual, but they recorded these cases as non-reportable on the AGE surveillance log. In addition, a reportable food worker case was not isolated by the medical staff for the required 48 hours after their last symptom, resulting in this person working while sick.

Potable Water-Striping

The combined distillate and permeate line from the evaporators and the reverse osmosis systems was not striped blue/gray/blue every 15 feet. The distillate line on the reverse osmosis unit #1 was striped blue for potable water up to the chlorine injection point.

Potable Water-Cross-Connection Control Log

The cross-connection control log indicated air gaps for the tank production and bunker tanks for chlorine, but upon inspection, these tanks and air gaps had been removed. Also, the description for the mechanical device on the cross-connection control log stated 'F.W. transfer to technical' on deck C in the engine room. According to staff, the backflow prevention device was for the water from the potable water tank going to the technical tank. Also, another description of a mechanical device stated 'distillate delivery', but according to staff this device was for the evaporator water that had already been chlorinated to the distillate tank. Because of the non-specific information given about these backflow prevention devices some staff were not sure of their location or purpose.

Potable Water-Engine Room

There was a small cooler with a small amount of water left that was stored on a soiled deck next to evaporator #2. Workers were using the nearby cups to drink water out of the cooler while working in the engine room. There were more than 20 workers in the area. The cooler and the cups were removed.

The deckhead in the candy storage room had small plenum spaces for ventilation and was difficult to clean.

The toilet for the kids less than six years old was greater than 11 inches from the deck and the handwashing sink was greater than 22 inches from the deck in the toilet room across from the reception area. A step stool was not in the room or available.

There was no flow meter installed and no turnover rate calculated for this RWF. Also, the bather load was calculated using the flow rate from the theoretical pump capacity.

Medical-Acute Gastroenteritis (AGE) Surveillance Log

A food worker had an onset of symptoms on December 2 at 9:30 am with two episodes of diarrhea and stomach cramps, but was recorded as non-reportable on the AGE surveillance log. There was no documentation indicating whether their symptoms were above normal or not. This individual was still in isolation during the inspection and was given antidiarrheal medication. No underlying illness was recorded.

On December 2 at 6:00 am, a non-food worker had an onset of two episodes of diarrhea and stomach cramps, but was documented as non-reportable on the AGE surveillance log. There was no documentation stating whether these symptoms were above normal or not. This person was isolated for 24 hours and was given anti-diarrheal medication. No underlying illness was recorded.

A food worker had four episodes of diarrhea and a headache on November 22 at 8:00 am, but did not report to medical until 5:05 pm on the same day. According to their work record, this individual did not work while sick.

A food worker had an onset of two episodes of diarrhea on November 28 at 6:00 am. The medical records indicated this was above normal for the individual, but this individual was recorded as a non-reportable case on the AGE surveillance log.

Medical-Crew Isolation

A food worker had an onset of two episodes of diarrhea on November 28 at 6:00 am. The medical records indicated this was above normal for the individual, but this individual was recorded as a non-reportable case on the acute gastroenteritis (AGE) surveillance log. This person was isolated until November 30 at 10:05 am, but this was less than 48 hours after their last symptom. According to their work record, this individual worked on November 30 on and off all day from 10:37 am until 11:15 pm when they still should have been isolated.

On November 4, there was one non-food worker who had one loose stool and stomach cramps, but was recorded as non-reportable on the AGE surveillance log. There was a note in the medical records indicating the symptoms for this person were above normal. This individual was isolated for 24 hours.

There was an open penetration in the water line that used to be connected to a piece of equipment that had been removed. This pantry was used for both food and medication storage.

A large round serving tray with clean china on top was set directly on the handwash station as staff removed the items from the tray to stage the breakfast service. There was no way to access or use the handwash station during this time.

The metal clamp holding the forward, counter model ice dispensing tube in place was heavily corroded. The clamp made contact with the ice exiting the tube.

There was a can of whipped cream set on a counter with a time control sticker attached with the discard time, but the time control plan made no note of cold foods on counters as being on time control, unless those foods were in temporary ice baths.

Galley-Time Control

In the posted time control plan the ovens, hot plate warmer, boilers, tilting pans, and work tables were listed as time control units, but no specific numbers were given and staff stated this was for all such units. Of these there were only time control labels attached to work tables and the portable hot trolleys. Additionally, the foods cooking inside some of the units were tomato sauce, chicken stock, beef stock, and jerk sauce, which were not going to be served immediately, but would be cooled and then reheated, according to staff. Other foods such as pork roasts in combination ovens and bean soup in steam kettles were served from these cooking units within the four hour period of service.

Galley-Port Hot Service Line

A live adult cockroach was observed in the basin of the #4 bain marie, which was clean and dry at the time of inspection.

Galley-Decks and Deckheads

The older decks in the hot galley and dishwash areas were chipped and had recessed grout in areas that made cleaning difficult. The deckhead had old food residue and dirt debris on the surface, with a concentration in the seams between the deckhead panels. The deckhead had an oil residue along crevices and profile strips, indicating a possible oil or grease product on top of the deckhead panels.

There was an excessive amount of oil droplets along the deckhead over the end of the hot service line near bain marie #4 and near deckhead access hatches that were directly over the food display/pick-up counter.

The deckhead throughout this area had a food and dirt residue build-up.

The final rinse spray nozzles in the upper final rinse manifold of the in-use conveyor glasswash machine did not have an effective spray pattern and the far right final rinse spray nozzle on the same upper manifold was slowly dripping water. Staff repaired the machine during the inspection.

Galley-Main Galley - Pastry

There was a continuous water leak in at least two sections of the in-use conveyor pot wash machine in the lower side exterior panels, with water pooling on the deck near the clean end of the machine.

Other-Bonsai Sushi Preparation Counter

The counter model sushi showcase refrigerator was not set on legs at the counter and was not sealed to the counter top. This long refrigerator was not easy to move so the area underneath could be cleaned by one person. Staff sealed the lower edge to the counter top during the inspection.

The vent for the backflow prevention device on the potable water supply pipe to the handwash sink was continuously leaking water.

The single light fixtures over the bank of steam kettles were heavily soiled with an encrusted debris. Some of the light fixtures had exterior labels that were peeling.

Numerous deckhead panels were repaired with spot welds that were not polished and left a very sharp edge that made cleaning difficult. Some areas had several panels hanging down, but still connected from the deckhead. The machine was immediately stopped for draining and cleaning. All ice in the front bar chests was also removed and the chests were cleaned.

A female passenger was walking in the hallway, lost her balance due to the movement of the CARNIVAL DESTINY fell forward and landed on her left wrist. She reported to the ship's medical center where an x-ray revealed a comminuted radial fracture to the left wrist. She disembarked the vessel in Ocho Rios, Jamiaca, for further treatment and flew back to Colorado.

Material Failure (Vessels)

While transiting from Key West to Cozumel the Auto-pilot failed and that caused the ARPA to also fail. Destiny had 2 operational radars and 2 GPS units for navigation. The vessel does not use an ECDIS system but uses up to date paper charts. The vessel added an extra helmsman to the scheduled watches to steer manually until they returned to Miami for repairs.

While moored in Miami, an Adult male crewmember (non U.S. citizen) was walking and talking to another crewmember when he stepped in the open space of a removed deck grill plate. The crewmember fell into the open space and sustained multiple rib fractures, fractures to the L1 and L2 spinal discs, and a contusion to his kidney. He was taken to a local hospital where he remained for 2 weeks and was then sent home to India on medical leave for further treatment.

CARNIVAL DESTINY (O.N. 9070058): 29Dec2011, A female passenger tripped over the metal threshold and fell to the balcony floor while entering cabin balcony. Passenger sustained left ankle fracture. Passenger used a wheelchair during an onshore excursion the following day. Injured passenger remained onboard to complete the voyage.

A female passenger was in cabin 6124 unpacking her luggage and turned to walk towards the bathroom when her foot got caught in the bed sarong , causing her to fall and fracture her right wrist. The passenger went ashore in Ocho Rios and had a cast put on her wrist. She returned to the ship and completed the rest of the cruise, disembarking in Miami on December 8, 2011.

CARNIVAL DESTINY (O.N. 9070058): At about 10:00 pm, while minor guest was descending the carpeted steps (running) inside the Palladium lounge from deck 05 to 04 forward starboard side, accidentally she lost her balance, after descending the last step on deck 04 and subsequently, she hit her right lower leg against the metal edge of the boundary wall behind the light and sound control booth and she sustained a fracture injury.

Material Failure (Vessels)

Received notification that the #10 Lifeboat on the CARNIVAL DESTINY was found to be not in it's stowed position and damaged on the Port Bow. #10 Lifeboat was found with the L/B Davit arms extended and the navigational lashings still secured. It was determined that an unkown party had attempted to launch the #10 Lifeboat without properly releasing the lashings. The #10 Lifeboat sustained damage to the port bow where the lashing is connected to the vessel hull. Damaged consisted of a hole in the vessel's fiberglass hull above the waterline. Hole is approximately 65cm X 75cm. #10 Lifeboat was offloaded in Port of Miami and will be towed to Ft. Lauderdale for repairs. Class has attended the CARNIVAL DESTINTY and issued a Short Term SOLAS Passenger Ship Safety Certificate, valid from 10SEP11 until 09OCT11. Passenger count has been reduced from 4470 to 4320 to account for the missing lifeboat. //s// MST1

Passenger was medically evacuated to South Miami Hospital, treated and released. The level of this investigation has been downgraded to "data collection" as per MSM V Part A5 Section C.

A female passenger was found unresponsive by her spouse in cabin 6107. The ship's medical team attempted to revive her for 20 minutes unsuccessfully. She was pronounced dead by the Ship's Physician. PAX underwent a recent Kidney transplant and had numerous other medical conditions. Cause of death was from Cardiac Arrest. Downgraded to Data Collection due to cause of death from natural causes.

Vessel Maneuverability

While CARNIVAL DESTINY was along side in Nassau and during pre-departure tests of the propulsion motors, the starboard propulsion motor (PM2) would not respond to increases of RPM. The fault was followed by a trip of the PM2. During trouble-shooting, it was discovered that the encoder interface board that relays commands to and from the control system for speed measuring during operation became faulty, rendering the motor unresponsive. The encoder interface board was replaced with a backup which was also faulty. The vessel proceeded to Miami using only the port propulsion motor (PM1). While in Miami, a technician came on board and conducted further tests. A repair was unsuccessfully attempted using the two faulty boards to make one working board. A spare was located on a sister ship in Orlando and was dispatched to Miami. Upon the board replacement, the PM2 responded properly to control system commands. Vessel was cleared for departure by USCG and the respective Class Society.

PAX was found in cabin 9-136 unresponsive and consequently confirmed dead. PAX suffered from a medical precondition (liver failure with cirrhosis with vericeal bleed and hepatic encephalopathy). This condition was related to K/C/O Alcoholic liver cirrhosis. Activity was downgraded to Data Collection due to previous medical condition.
