Federal health inspectors from the Centers for Disease Control and Prevention inspect cruise ships twice a year, checking everything from kitchen equipment to dishwasher temperatures, and grade ships based on what they find. A score below 86 is considered failing. Cruise ships are also required to report all illness outbreaks affecting more than 3% of passengers or crew to the CDC. Inspections and outbreaks from 2010 to the present are shown below.

The temperature of a tray of ham, cheese, and lettuce sandwiches was 70°F. The temperature of a tray with cut melon was 71°F. The trays were on a table in the technical office and not on time control. Both trays were discarded.

The four hot tubs had a single chlorine and pH monitoring line drawn from a common overflow pipe. As a result, there was no automated free halogen residual and pH monitoring for each single hot tub.

The analyzer was malfunctioning since it was not analyzing the chlorine and pH levels properly. The analyzer was reading chlorine levels of 2.5 ppm and pH of 7.5. Manual tests taken from the water going to the analyzer from the pool were measured by the staff at 0.66 ppm of chlorine and pH of 6.8. These levels were consistent with what was found on manual tests at the pool itself. The chart of the chart recorder showed steady chlorine levels of 2.5 ppm and pH of 7.5 for hours.

When measured by the inspector and staff member from different parts of the pool, the chlorine levels were 0.31, 0.32, and 0.30 ppm and the pH levels were 6.6, 6.7, and 6.9. The pool was open at the time of the inspection and closed immediately after the findings.

Numerous piece of food equipment throughout the galley's were noted as out of order and awaiting repair parts. Examples included the pizza deck oven in the Garden Cafe Central Galley, numerous microwave ovens in Main Galley, stacked ovens in Main Galley, and a vertical chopper in the buffet prep pantry.

Medical-

A Guest Services Attendant experienced acute gastroenteritis (AGE) symptoms on 5 June at 19:20 and additional symptoms on 6 June at 08:45. She was released from medical isolation on 6 June at 19:20, less than the required 24 hours isolation. Her time card indicated she did not work 5, 6, or 7 June. Since she was release from medical isolation on 6 June, her onboard activities could not be determined for 7 June. An Entertainer experienced AGE symptoms on 6 June at 11:00, but did not report to the medical center until 18:23. Her work hours could not be determined since this position was not considered a seafarer. According to her 72-hour self-administered questionnaire she went to the Garden Cafe at 15:00.

Potable Water-

The list of backflow prevention devices did not include devices for passenger and crew cabin toilets.

The grill chef's did not demonstrate any food safety knowledge while they were in active preparation of the food for the ship's passengers sitting at their grill station.

A deck pallet had been placed on its side and the bottom of pallet was in direct contact with a box of bottled beer. The pallet was immediately moved.

There was one large fly in this area.

The deck tiles in front of the combination ovens in the hot galley had missing and recessed grout. The vessel has a comprehensive plan for deck repairs and this area was listed on it.

Bar-Shakers

Wrapped straws and cocktail napkins were stored among office supplies such as a staplers and signs in the storage locker next to the Bistro galley entrance. The food-service items were moved.

The glasswash machine had been out of order since 18 November. Spare parts had been ordered and were expected to arrive by the end of December.

The vent on the backflow prevention device for combination oven 551 212 000 was plugged with a screw-in plug. The device was replaced.

A crew member had placed the electronic end of a thermometer in his mouth while using a squeegee. The crew member was instructed that this was not an acceptable practice.

The wrapping on one set of soda lines was split in the technical compartment with the CO2 tank.

According to the staff, hamburgers could be cooked to order. The posted consumer advisory indicated that eggs could be cooked to order, but there was no mention of hamburgers being cooked to order. The buffet was open for lunch and hamburgers were being cooked. Staff found the sign for hamburgers and posted it in the station.

There was what appeared to be a small hair on the right, lower side of the ice-making compartment of the right ice machine. The hair was removed.

The shelves in this storage room were made of galvanized steel and had holes, seams, and other difficult to clean features. Beverages were stored in this room.

Children Area-Illness Exlusion Policy

The illness exclusion policy posted at the entrance to the child activity center was blocked by a large portable sign. The sign was moved.

A Photographer had an onset of acute gastroenteritis (AGE) symptoms on 30 November at 18:00, but did not report to the medical center until 2 December at 21:52. Her time card indicated she worked on 1 December from 06:30 - 10:30 and 17:00 - midnight, and also on 2 December from 16:00 - 21:00. An Assistant Cook had an onset of AGE symptoms on 1 December at 13:30, but did not report to the medical center until 4 December at 16:55. Her time card indicated she worked on 1 December from 18:00 - 23:00, on 2 December from 08:30 - 10:00 and 18:00 - 23:00, on 3 December from 08:30 - 10:00, 16:00 - 21:00, and 21:30 - 23:00, and on 4 December from 09:00 - 12:30 and 16:00 - 17:30. A Utility Galley had an onset of AGE symptoms on 2 December at 20:00, but did not report to the medical center until 4 December at 18:50. His time card indicated he worked on 2 December from 17:30 - 23:00, on 3 December from midnight - 03:00, 03:30 - 05:00, and 17:30 - 23:00, and on 4 December from midnight - 03:00, 03:30 - 05:00, and 17:30 - 23:00. Another Utility Galley had an onset of AGE symptoms on 6 December at 05:30, but did not report to the medical center until 19:35. His time card indicated he worked 17:00 - 18:00.

His time card indicated he worked on 2 December from 17:30 - 23:00, on 3 December from midnight - 03:00, 03:30 - 05:00, and 17:30 - 23:00, and on 4 December from midnight - 03:00, 03:30 - 05:00, and 17:30 - 23:00. Another Utility Galley had an onset of AGE symptoms on 6 December at 05:30, but did not report to the medical center until 19:35. His time card indicated he worked 17:00 - 18:00.

Potable Water-Bunkering Records

On 29 November, potable water bunkering started at 11:45. According to the written records, from 13:10 - 15:10, the chlorine level was less than 2.0 ppm. The records indicated the chlorine level was 1.04 ppm at 13:10, 1.10 ppm at 14:10, and 1.10 ppm at 15:10. At 16:10, the chlorine level was 2.10 ppm and production stopped at 16:45.

Potable Water-Deck 1, Compartment 14

Prior to the backflow prevention assembly for hot water to technical water, the water line was not striped to indicate potable water. By the end of the inspection, the water line was striped blue.

Four previously cleaned plates and one rice bowl on the buffet line for crew members had dried food soil on them. Soiled silverware was also found in the crew mess line utensil dispensing bin. All plates and bowls were removed immediately for re-washing.

The final rinse section of the dishwasher has two final rinse manifolds. The second manifold was not spraying water from its final rinse nozzles. The manifold was removed and remounted creating a good connection to the hot water supply line and a good spray pattern was restored.

Old food soil was noted on the large vegetable slicer chutes stored in the clean area particularly below the nuts on the interior of the chutes. Vegetable processor parts stored in the clean area had large dried vegetable particles on them. These parts were sent for rewashing.

The stream of water was straight down from the nozzle and steam could be heard coming from the nozzles. The final rinse gauge indicated the water temperature ranged from 197° - 207°F in the pipe rather than a maximum permitted temperature of 194°F.

Old milk residue was found in a previously cleaned milk dispensing unit tube of a cappuccino machine. The stainless steel pan where the milk tube started had been already cleaned.

There was a data logging system used to record all pH and chlorine residual readings from each of the Recreational Water Facilities (RWF's) on board, but in reviewing the manufacturer's documentation on this data system there was nothing to verify that it had certified data security features. Staff were able to copy the data down to a computer file and produce an Excel spreadsheet of the information.

Potable Water-Tank Coating

On 28 April portside #17 potable water tank was recoated with epoxy paint, but there was no information in that record detailing the application, drying and curing, and there was no statement that the tank coating was applied, dried, and cured according to the manufacturer's specifications.

There were two live fruit flies observed on the top loading edge of the out of order potato peeler.

Medical-Crew Gastrointestinal (GI) Illness Reporting

An assistant pastry cook had onset of GI illness symptoms at 2 am on 26 April and did not notify the medical center until 10:40 am the same day. A cadet engine staff had onset of GI illness symptoms at 6 pm on 3 May and did not notify the medical center until 9:35 am on 4 May. A youth counselor had onset of GI illness symptoms at 12 noon on 4 April and did not report to the medical center until 5:25 pm the same day. An assistant waiter had onset of GI illness symptoms at 10:45 am on 22 March and did not report to the medical center until 6 pm that day. The review of work history indicated that none of the above employees worked during their illness, but there is always a medical staff member on duty at the medical center and all employees are expected to notify medical when GI illness symptoms occur.

In reviewing the reporting of gastrointestinal (GI) illness cases among crew at the medical center, one assistant cook had reported to the medical center on 15 November at 4:20 pm with GI illness, but his diarrheal symptoms began on 13 November at 7 pm. Another case was a bartender who reported to the medical center on November 18 at 5:40 pm, but his diarrheal symptoms began on 16 November at 5 am. The bartender indicated his last diarrheal symptom was at 8 am on 18 November.

In response to the outbreak, Norwegian Cruise Line and the crew aboard the ship reported the following actions:Increasing cleaning and disinfection procedures according to their outbreak prevention and response plan,Making announcements to notify onboard passengers and crew of the outbreak, encourage case reporting, and encourage good hand hygiene,Collecting stool specimens from passenger and crew gastrointestinal illness cases for testing by CDC,Making twice daily reports of gastrointestinal illness cases to the VSP,Sending Norwegian Cruise Line public health personnel to join the ship at St. Thomas to assist the onboard management with infection control response plan,Is consulting with CDC on plans for their comprehensive sanitation procedures in Miami, FL on April 6, 2015, including: alerting passengers for the next voyage of a delayed embarkation because of the current cruise gastrointestinal illness levels,early arrival plans for the current cruise to allow for early disembarkation of the current passengers,planning controlled disembarkation for active cases to limit the opportunity of illness transmission to well guests,providing additional cleaning personnel to complete a thorough public area, and stateroom cleaning and disinfection protocol implementation, andplanning for sanitation of terminal and transport infection control procedures.Two CDC Vessel Sanitation Program environmental health officers will board the ship in Miami, FL on April 6, 2015 to conduct an environmental health assessment and evaluate the outbreak and response activities. Specimens are being collected and will be tested by CDC to determine the causative agent for this outbreak.

Lifesaving

In addition to the life jackets required by reg. 7.2, every passenger ship shall carry life jackets for not less than 5% of the total number of persons on board. These life jackets shall be stowed in conspicuous places on deck or at muster stations. The number of life jackets prescribed on the lifesaving and evac. plan does not match the number of life jackets on deck. 74 SOLAS (09) III 22.2.1

Operations/Management

Drills shall include excercising the procedures required for the deployment of an MES system up to the point immediatly proceeding actual deployment of the system. MES crews are not training to the point of deployment. 74 SOLAS (09) III 19.3.3.8

Fire Fighting

Escape Routes shall be maintained in a safe condition, clear of obstacles. FSD 6-5-0-73, FSD 5-7-076, and FSD 4-7-128 were blocked by locked doors. 74 SOLAS (09) II-2 13.1.2 *Vessels crew unlocked all doors labeld as emergency exit doors and will be removing locks in the future. Awaiting a corrective measures letter from the company.

Personnel

Every crew member with assigned emergency duties shall be familiar with these duties before the voyage begins. Vice Commanders on lifeboats were unfamiliar with lifeboat procedures. 74 SOLAS (09) 19.2.1

Electrical installations shall be such that the safety of passengers, crew, and ship from electrical hazards will be ensured. There is a dead end cable in FST-14-4-11. (2004 SOLAS II-1/40.1.3)

NORWEGIAN PEARL (O.N. 8001150): On 19Feb2012, while in the Port of Miami, an adult male passenger was standing outside the stateroom of his friend. While waiting, he was leaning against the door post with his right hand between the post and the door. When his friend exited the room and closed the door it caused an impact injury to the victim's right thumb, partially amputating the digit. He was medically evacuated to Jackson Memorial Hospital while still in port.

An adult male passenger was found by his wife laying on the bed in their cabin and he was not breathing. The medical team was immediately notified and responded. Attempts to revive the pax were not successful and he was pronounced deceased by the ship's physician. Cause of death was from Myocardial Infarction. Downgraded to Data Collection due to a pre-existing medical condition and death from Natural Causes.

An adult male passenger was reportedly having trouble breathing. The oncall Nurse was notified and responded. Upon arrival she observed the passenger was not breathing. She initiated the Emergency Medical call and began CPR. The ship physician and medical team arrived , and continued CPR with no response. The Ship Physician pronounced the passenger deceased due to Respiratory/Cardiac Arrest with contributing factors of pre-existing medical conditions. Downgraded to Data Collection due to death from Natural Causes.

NORWEGIAN PEARL (O.N. 8001150): At 0810 on 29Jan2012, crewmember was participating in the Marine Evacuation System training. He was descending starboard side aft deployment station chute into the life raft in the water below. Upon exiting the chute, and landing on his feet in the life raft, his left ankle twisted and this caused a fracture. Crewmember taken to local hospital for treatment. Note: Marine Employer did not conduct post-casualty drug & alcohol testing on the crewmember directly involved.

NORWEGIAN PEARL (O.N. 8001150): During passenger muster drill, an elderly male passenger's foot got caught and he fell forward, suffering from a contusion (bruise) to his right hip. Passenger was evaluated by ship's medical staff and was disembarked to a local hospital in Miami, FL. Passenger received pin during hip surgery as result of fracture sustained during fall. No drug or alcohol testing was conducted on the ship's crew as no action or inaction by the crew contributed to the occurrence of this casualty.

The passenger did not respond to stimuli and was pronounced deceased by the ships physician. 02/07/2012: Rcvd Medical Examiner report, cause of death was from Hypertensive and Valvular Heart Disease with contributory causes of Micronodular Cirrhosis of the Liver and Chronic Pancreatitis. Downgraded to Data Collection due to death from natural causes.

A Male passenger tripped over a small table in cabin 1000 while walking towards the entry door. The fall resulted in a fractured left femur. Pax disembarked the vessel on 4/25/2011 in Puerto Quetzal, Guatemala to seek further medical attention at a local hospital and then fly back to the U.S. Though this is defined as a serious marine incident (SMI) no drug or alcohol tests were conducted for the NORWEGIAN PEARL's crew members as no actions or inactions contributed to the passenger experiencing the marine casualty.

ON April 3, 2011, a female passenger slipped and fell entering the restroom on deck 12. During a port visit in St Thomas on April 6, 2011 she arrived back at the ship wearing a sling. The PAX reported to the ship's medical center on April 8th where she was examined and found to have a fractured left elbow. She remained onboard until the ship arrived in Miami where she disembarked.
