Stroke is a major public health problem leading to death, disability, and dementia. African Americans and people who live in the Southeastern region of the United States (US) experience a higher rate of stroke compared to people of other races and US regions; however, stroke is very preventable and adequately treating hypertension has the biggest effect on stroke prevention. This research seeks to test a mobile- phone and home monitoring based strategy for improving the currently poor control of hypertension among patients (half of whom will be African American) at high risk for future stroke in South Carolina, a State with an especially high rate of deaths from stroke.
