Dr. Cogle's company would like for me to review GBM cases on as needed basis. They will provide medical records regarding the patient's Glioblastoma diagnosis and Next Generation Sequencing report and then would ask for me to comment on treatment options. I am attaching the draft of the agreement since I don't plan to sign anything until I receive approval from eCOI
This is an as needed agreement. The client has not stated how many days will be needed. Payment is hourly. My plan is to not take time off from work because I will do this consulting work in the evening so as not to do during normal work hours. The term of the agreement is for one year which is what I reflected in the start and end date of services I placed Anticipated pay in the 12 months as $2000 because the company is stating a fee of $500/hr. They have not told me how many cases I will be asked to review. I am assuming that a case may take me 1 hour to review so I am giving my best guess
