Training of clients in home setting on how to fit, apply and use specialized edema pumps. None of the patients have been in my care, nor are they patients of this facility.
I have only recently been introduced to this company and agreed to go on visits with staff for training. I have completed training, have completed online competencies in past couple of weeks and have decided to move on with this company on a PRN basis to see patients in evenings and/or on weekends. This company will not refer me any patients I have ever had any contact with. Therefore, I am filling this out prior to seeing 4 new referrals I just received within the past few days. Per visit basis. Number of referrals is not set and appears to be a few per month..

Notes: When a more specific filing date is not available for an individual financial disclosure or conflict of interest form, we use the year the form was filed. If the year was not disclosed, we report the range of years covered by our public records requests. In a few cases, a start date was provided instead of a filing date. In those cases, we use the start date instead.
