Issue:
Birmingham VA Medical Center (BVAMC) Privacy Officer (PO) received notification via email on 07/11/13 that on 07/10/13, a BVAMC patient reported for an appointment in the BVAMC Blue Clinic. The patient had a copy of medical records to review and noticed that the pathology report from 05/31/13 had another patient's name listed. Disclosure of the incident was discussed with the patient by BVAMC Nurse Manager and a complaint filed with the BVAMC Patient Advocate. The Nurse Manager attempted to contact the Lab Supervisor but was unsuccessful. Patient Advocate has notified the Lab Supervisor and requested a response.
Outcome:
07/11/13: One patient will be sent a letter of notification.
