Issue:

Veteran A requested a copy of some of his medical record information. Veteran B received Veteran A's medical information in the mail. Veteran B opened up the envelope with medical records inside, noticed the records were not his medical records, closed the envelope and provided the medical record data to a Veteran Service Officer. The Veteran Service Officer contacted Release of Information and the officer will be returning the records. Update: 01/31/12:Veteran B will be sent a letter offering credit protection services, as his SSN and medical information was disclosed.NOTE: There were a total of 31 Mis-Mailed incidents this reporting period. Because of repetition, the other 30 are not included in this report, but are included in the "Mis-Mailed Incidents" count at the end of this report. In all incidents, Veterans will receive a notification letter and/or credit monitoring will be offered if appropriate.

Employee was reminded on the importance of ensuring accuracy when mailing out personal identifiable information.
