Issue:

Veteran A contacted this Privacy Officer (PO) to report their protected health information (PHI) was displayed on an overhead projector during an educational training class for Veterans. Upon discussion of the incident, it was determined the Veteran's demographic information, through VistA, was displayed on the overhead proje400 Veterans Avenuector while the instructor checked to see if the Veteran had an appointment for the class. One of the Veteran students noticed the overhead display, and told the Veteran. The Veteran informed the instructor, and the instructor immediately exited the demographic display. It was determined there were three other students in the training class and the Veteran's demographic information was displayed for approximately five seconds. The reporting Veteran was concerned their appointment schedule was viewable and showed they had a future appointment with "MH". The Veteran did not want anyone to know they were being seen in mental health.

Outcome:

08/27/14: The Incident Resolution Service Team determined that the Veteran will receive a HIPAA letter of notification, since the PHI was visible as evidenced by the fact that one of the other Veterans informed Veteran A.
