Issue:

A Veteran reports that her ex-boyfriend (VA employee) has either accessed her medical record or had a co-worker access her record as the ex-boyfriend told her everything about her past appointments that she shared with her provider.

Outcome:

06/10/13: The ex-boyfriend no longer works at the VA however he still could have another employee accessing the Veteran's information without a need to know. The Privacy Officer (PO) sent an email the employee's supervisor to investigate. 06/17/13: The PO has access to some additional files that she needs to search to see if this specific employee was accessing the record as part of her job or not. She has contacted the employee's Supervisor and Service Chief and will be working with them to talk with the employee and find out the details. 06/24/13: The PO is still reviewing the files that were provided, however the employee in question is now out of the office for an extended period and we are not able to discuss this with her. From what the PO has reviewed so far, it clearly appears the employee was in the record without a need to know. The ex-boyfriend employee who the PO thinks she was sharing the information with no longer works at our facility. 07/15/13: The Veteran will receive a letter of notification since the employee was in her record without a need to know.
