Issue:

Patient A went to the Emergency Room on 03/15/13 and when he got home, he realize he had Patient B's armband on his arm. He called the Patient Representative on 03/18/13 because he was concerned that his medical information may have been written in the other patient's record. Patient A has not returned the armband. Update: 03/26/13:The armband has not been return to us. We did get the name of the patient whose armband he had. That patient did not leave the facility with an armband. The Patient states that he lives with his grandchildren and they may have taken it to play with, the children are minors. Therefore Patient B will receive a letter offering credit protection services.

Outcome:

Staff was reminded that for patient safety they double check each armband before placing and upon placing it on the patient. They were also told that they must remove the armband from each patient as they leave the Emergency Department.
