Issue:

Based on staff interview, clinical record, and administrative document review the facility failed to keep Protected Health Information (PHI) confidential when Patient 1's name and account number were mistakenly sent to Patient 2 along with Patient 2's billing statement of services provided at CMRC.This failure placed Patient 1's PHI at potential risk for unauthorized use.Findings:On 9/5/14 at 10:58 a.m., Staff 1 stated that on 7/1/14 the facility became aware of a possible privacy breach. The facilities internal investigation reveled on 6/13/14 Patient 1's PHI was mistakenly sent to Patient 2 in the form of a billing statement for services.On 9/5/14 at 10:58 a.m., Staff 1 stated the PHI breached included Patient 1's name , and account number.On 9/5/14 the facility policy and procedure titled "HIPPA General Rules for the Use and Disclosure of PHI" dated 04/18/2012, contained the following documentation: "III. A. It is the policy of CMC to protect the privacy and security of patient information and to comply with applicable laws and regulation."
