Issue:
A providers vehicle was broken into this morning outside of the VA campus. A bag containing the providers VA ID badge, day planner and other personal items was stolen from the vehicle. A census sheet with VAPAHCS Recovery Program header containing 47 patients full names is also missing. Six full names on the list has full SSNs and 41 full names just have the last 4 SSNs. No other protected or sensitive information was listed on the document.
Outcome:
11/05/13: Six (6) Veterans will be sent letters offering credit protection services and Forty-one (41) Veterans will be sent a notification letter.
