Issue:

Through an access request of a Veteran/Employee, it was noticed that a fellow employee had accessed his medical records prior to his placement with that particular unit. The accessor has no knowledge why she was in his record but a fellow employee has acknowledged overhearing her talking to other employees about reviewing his medical information. Update: 11/07/12:Veteran/Employee A will be sent a letter offering credit protection services.11/20/12:Appeal granted on credit protection services, but notification needs to be done. Also the facility needs to ensure that administrative action needs to take place due to repeat offense.

Outcome:

Employee is to receive a reprimand for her actions. HR waiting to hear back from Supervisor. Notification letter sent to wronged employee several weeks ago. Copy filed in system for verification of mailing.
