Issue:
Based on interview and record review the facility failed to ensure the confidential treatment of protected health information for Patient A, when Patient B was given an imaging report intended for Patient A resulting the unauthorized release of Patient A's confidential health information.During a phone interview on June 10. 2015 at 2:00 PM, with the Emergency Department Physician (EDP) said, they cut and paste the imaging report results (the image results) onto the Emergency Department reports. He's embarrassed he let this happen and this was a HIPAA (Health Insurance Portability and Accountability Act) violation. A review of Patient A's imaging report results that was breached dated on April 1, 2015 included: x-ray (A quick painless test that produces images of the structures inside your body) results, patients name, date of birth, age, sex, account number, Doctors name, medication dose, and frequency.
