Issue:
Veteran reported receiving a package in the mail containing Veteran/Beneficiary Claim for Reimbursement of Travel Expenses meant for several other Veterans. Three of four Veterans' information that was at risk included full name, SSN, DOB, and home address. One of the four Veterans' information that was at risk included, full name, last four digits of the SSN, home address and future appointment dates.
Outcome:
11/26/14: The Incident Resolution Service Team has determined that three Veterans will be sent letters offering credit protection services and one Veteran will be sent a HIPAA notification letter.
