Issue:
The Privacy Officer (PO) received an email message from the VA Health Resource Center (HRC) that they were contacted by Veteran A who received a mailed VA bill for Veteran B with a similar name. The information contained in the bill included the Veteran B's full name, address, phone number, full SSN, medications and co-pay balance for each medication. Update: 03/01/12:Veteran B will be sent a letter offering credit protection services.
Outcome:
MCCR Revenue Coordinator/Supervisor will educate staff regarding the importance of verifying the address for a Veteran before any returned mail is sent to a new address.
