Issue:
Based on interview and record review, the facility failed to ensure discharge instructions for Patient A were not sent home with Patient B. This failure resulted in the unauthorized disclosure of Patient A's protected health information (PHI), which created a potential for unauthorized use.Findings:On March 29, 2012, at 2:45 p.m., the Compliance Specialist was interviewed. The Specialist stated discharge instructions and record documents for Patient A were sent home with Patient B. The instructions contained Patient A's name, age, date of birth, medical record number, account number, visit highlights and medication information.On March 29, 2012, the facility policy and procedure titled, "Information Privacy," was reviewed. The policy indicated, "...Unauthorized or unlawful use: is the sharing, utilization, examination or analysis of PHI within EMC if that information is not needed by each of the individuals involved in that patient's treatment..."
