Issue:
Event Location: CLC The ACN of Extended Care reported concerns of a possible privacy violation. Her concerns were based on the fact that a Union Representative had presented a copy of a CLC resident's progress note to the Chief of HIMS. It was originally thought that Union Representative, who is a newly appointed Union Steward and a nurse at the facility, may have printed the note from the resident's CPRS records; however, it was later reported to the Labor Relations Representative that (allegedly) a CNA, who works at the CLC, printed the document and provided it to him. Preliminary findings are that the employee presented the patient's progress note to Union for representation purposes. Update: 10/27/11:One Veteran will be sent a letter offering credit protection services due to full name and full SSN being disclosed.
Outcome:
Per Labor Relations Representative, education was provided to the Union Representative and the employee who shared the patient's information. They were instructed on the procedures for requesting patient information for Union purposes.
