Issue:
Veteran A contacted the Release of Information department and reported that he received a disk with Veteran B's health information. Veteran A will be returning the disc to the facility, the data at risk will be reviewed at that time
Outcome:
05/07/14: The disk has been retrieved by Release of Information staff and the it was confirmed that Veteran A received Veteran B's information. Privacy Officer is changing ticket from complaint to incident. Veteran B' name, address and protected health information (PHI) was compromised. 05/12/14: The Incident Resolution Team determined that Veteran B will receive a HIPAA letter of notification.
