Issue:
On 03/20/13, Veteran A reported for his clinic appointment, but discovered that he did not have one scheduled in the system. On closer examination of his appointment letter, the clerk and provider discovered that he was given an appointment letter for another patient, Veteran B. There were 2 documents in total. They contained the following: 1. An appointment letter with Veteran B's full name, address, appointment information, and his last initial and last 4. and 2. An appointment list with Veteran B's full name, full SSN, and future appointments. The documents were recovered and turned over to the Privacy Officer (PO) to investigate. . Update: 03/21/13:Veteran B will receive a letter offering credit protection services.
Outcome:
Documents returned to the medical center Privacy Officer. Issue discussed with provider and clinic coordinator.
