Issue:

Patient came to medical center for an appointment and handed the VA employee a list that he says someone at the CBOC gave to him. Due to the age of the veteran it is believed that he may have picked the paper up off of a desk without realizing he had it. He said that someone gave it to him. The list was a clinic appointment list. It contained the names of seven (7) Veterans plus his name. Along with the names was the SSN, phone number and means test and co-pay status. It appears that the first three appointments of the day had been checked off. The Administrative Officer (AO) for the CBOC is investigating but it is believed that one of the clerks may have accidently handed it to him or he may have accidently picked it up off a provider's desk. Investigation ongoing. Veteran is 80+ years old so risk of misuse is highly unlikely. Update: 11/10/11:Seven Veterans will be sent a letter offering credit protection services due to full name and SSN being disclosed.

Outcome:

Staff will maintain clinic schedule list on clip board to insure it is not picked up by accident.
