Issue:

A Louisville VA Veteran patient (Veteran A) called and stated that his results letter from his Primary Care Clinic had been mailed to another Veteran's residence in Ohio. Veteran A could not be located in the system, it is uncertain how this happened exactly. The complainant (Veteran A) indicated that the information in the window of the envelope was his name, but the other Veteran's address. The other Veteran is deceased, but the widow of the Veteran returned the information to the complainant (Veteran A) by mail. The letter was postmarked 3/23/11 and he received it back to him on 4/2/11. The mismailed letter included Veteran A's full SSN. Update: 04/04/11:Veteran A will be sent a letter offering credit protection services.

Outcome:

I sent an e-mail to the Service Chief, Administrative Assistant, Nurse Manager and the physician. I indicated that re-education to the staff was needed and to let me know once this had been completed. I also indicated that I was available to provide training to staff if needed.
