Issue:

Veteran A reported he was contacted by Veteran B two weeks ago that he had received a document from the VA with both of their protected health information (PHI) on it. They arrange to meet at the VAMC today to hand off a copy. The Veteran who received it today reported it to the Facility who then contacted the Information Security Officer (ISO). Based on preliminary investigation it looks like a system error that printed out a single page outpatient request form with two different patients' PHI on it. The ISO is currently working with Service Delivery and Engineering (SDE) and others to see how this happened.

Outcome:

12/17/14: The Incident Resolution Service Team has determined that Patient B will be sent a letter offering credit protection services.
