Issue:

A VA employee took the applications and medical records for five applicants to the VA Summer Sports Clinic home with them. The information was left in the employee's car. The employee's car was broken into and the information was stolen. VA employees searched and dug through dumpsters in the area where the information was stolen but were unsuccessful in recovering the information. Information at risk includes the Veterans contact information, full social security number, and medical information. The identities of the impacted Veterans is known and the interim director of the summer sports clinic will attempt to contact the Veterans to provide them with a notification that the information was stolen. Update: 06/04/12:Five (5) Veterans will be sent letters offering credit protection services.

Outcome:

Training was held with the staff to educate them on the proper handling and transportation of VASI.
