Issue:
Veteran A received Veteran B's medications at the pharmacy window. As soon as he discovered the mistake he brought the medications back to the pharmacy and they were destroyed. The information at risk included Veteran B's name and medication information. Update: 04/24/12:Veteran B will be sent a letter of notification.NOTE: There were a total of 14 Mis-Handling incidents this reporting period. Because of repetition, the other 13 are not included in this report, but are included in the "Mis-Handling Incidents" count at the end of this report. In all incidents, Veterans will receive a notification letter and/or credit monitoring will be offered if appropriate.
Outcome:
The pharmacy staff member received a verbal counseling and will review the privacy video.
