Issue:
Based on interview and record review the facility failed to ensure the confidential treatment of protected health information for Patient B, when Patient A was given discharge paperwork intended for Patient B resulting the unauthorized release of Patient B's confidential health information.During a phone interview on June 30, 2015 at 3:35 PM, with the Medical Assistant (MA) said, I was reading the discharge paperwork for Patient A and noticed the visiting summary was missing. I told the physician I needed the visiting summary for Patient A. I went back to look for physician and the physician was gone and I noticed a visiting summary document on the printer I assumed it belong to Patient A. I did not check the patients name on the visiting summary document. "It was my fault."A review of Patient B's visiting summary that was breached dated on May 29, 2015 included: Patients name, sex, date of birth, medical record number, home phone number, Doctors name, allergies, medication, height, weight, temperature, body mass index (A measure of body fat), BSA( Is the measured surface area of a human body),vital signs,diagnosis,and patient education regarding diagnosis.
