Issue:

A VA Social Worker self-reported that he was assisting a Veteran complete a Release of Information authorization and he put the incorrect Veteran's name and last four SSN on the form instead of the Veteran he was working with. After the Veteran signed the form the Social Worker provided the Veteran with a copy that he folded and placed in his pocket. As soon as the Veteran left his office he noticed his error and immediately stopped the Veteran and told him they needed to complete another form. He confiscated the form he had given to the Veteran in error. We don't believe the Veteran even looked at the form or he would have returned to his office since he needed the form for business purposes.

Outcome:

01/12/15: The Incident Resolution Service Team has determined that there was a policy violation. While there was an unauthorized access/disclosure of data, it has been determined that the incident has a low probability of a risk of compromise.
