Issue:

Release of Information staff member mailed a Veteran's medical records via US mail to VA Regional Office and they cannot be accounted for. Update: 03/28/12:Correspondence clerk sent records on 12/1/11 and entered on the transaction "all records from 1995- present." The medical records were mailled out via US mail. There is no way of tracking since it was sent via US mail. Jan 10, 2012 Regional office informed us the that they had not received the records and did a search. It was confirmed on Jan 11, 2012 that they were unable to locate any records of Veteran. Due to full SSN and medical infor being lost/unaccounted for Veteran A will be sent a letter offering credit protection services.

Outcome:

Release of Information staff person mailed Veteran's medical record via US mail (unsecured delivery) to VA Regional Office and the records cannot be accounted for. After a thorough search the records still cannot be accounted. To prevent reoccurrence, the ROI Staff were reminded of the secure means of mailing Veteran medical records (by use of tracking system that is available); and if they fail to follow the proper procedures for mailing Veteran's medical records administrative disciplinary action can and will be recommended.
