On 09/11/12, someone dropped off a total of 18 plastic bags with patients' personally identifiable information (PII) on label adhesives in the alternate Privacy Officer's (PO) office. This person did not leave contact information and details. The labels contained patients' names, full SSNs, and dates of birth. Update: 09/17/12:The 18 patients will receive a letter offering credit protection services.09/18/12:The PO found some duplicates so the total number of patients to receive a letter offering credit protection services is 14.
