Issue:

Based on interview and record review, the hospital failed to report a breach of confidential medical information within five business days of discovering the breach. Findings:During an interview on 3/5/14, the Manager of Accreditation and Licensing stated that on 1/13/14 an off-campus clinic reported that over the preceding weekend eight desktop computers and two flash drives had been stolen. A faxed letter notifying CDPH was dated 1/27/14 at 3:38 PM stating that the computer records of the stolen items were being analyzed to determine the nature and extent of the protected health information.The hospital was six days late in reporting this probable breach of confidential medical information to CDPH.
