Issue:

This incident occurred May 12, 2014, during a Nursing Shift Report that was being held to discuss the status of the Veterans on that specific Ward to the oncoming shift of Nurses. During this Shift Report, it was noted that the Charge Nurse allowed a Veteran/Patient to accompany the Charge Nurse into this Shift Report in which twenty (20) Veterans medical information was discussed. No personal identifiers are used in the meeting, except the name of the patient being discussed. The Veteran/Patient was allowed to remain in the meeting the entire time. The Nurse Manager of this Unit did state that the Veteran that attended, does have diminished mental capacity and cannot be sure if this Veteran would be able to retain the information that was discussed.

Outcome:

05/19/14: The Incident Resolution Team has determined that twenty Veterans will be sent a notification letter.
