Issue:

The Privacy Officer received a phone call from the Clinic Manager at Ft. Knox who indicated she had just received a phone call from Veteran A. He reported to the Manager that he had received an appointment letter and on the back of the appt. letter was lab results for another Veteran B. Veteran A stated he tried to contact Veteran B, but could not reach him. We asked Veteran A to destroy the information and he agreed to do so. The Chief of the clinic was also informed of this mishap and she will be sending out a message regarding the printing of information and to ensure safeguards are in place. Also will make sure that extra care is taken when printing and mailing information. Update: 10/04/12:One Veteran will be sent a letter offering credit protection services.

Outcome:

Discussed with service chief the importance of safeguarding information. Especially when printing front to back material that could be mixed in with other paperwork of other Veteran's. Service chief voiced understanding and indicated that e-mails and training would be provided to all staff.
