Issue:

A patient met with the Acting Privacy Officer (PO) and noted that his Medical Information had been accessed by a VA employee that was related to his fianc'. He noted that he had not been seen at the VA; however, he had been in the Navy and Reserves in the past. The PO entered the electronic medical record program and found that the patient had been seen in the Emergency Room in 2001 for humanitarian treatment. The PO contacted Information Security Officer (ISO) and he investigated and confirmed that it was an Incident where the patient's record had been accessed by the employee. Update: 02/10/12:The VA employee was not involved in the Veterans care and did not have a job related reason to access the record. The Veteran will be sent a letter offering credit protection services.

Outcome:

Chief of Primary Care had the Employee complete the VA Training on Privacy and on Information Security even though the employee was currently compliant with that training. Certificates of Completion were to be turned in to the Chief of Primary Care today, 2/9/2012.
