Issue:

The disposition note written for active duty Patient A contained the personal information of active duty Patient B. The information contained Patient B's full name and full Social Security Number. Patient A turned the document into the Navy Liaison who contacted the ward. The ward Nurse then went to Navy Liaison to retrieve the documents and put them in the sensitive bin. The Doctor who wrote the note has requested the improper documents be removed from the records. The incorrect information is still in the incorrect Patient's chart

Outcome:

06/09/15: The Incident Resolution Service Team has determined that Patient B will be sent a letter offering credit protection services due to full SSN being disclosed.
