Issue:
Veteran A reported that two letters were sent to his home address. One letter was intended for Veteran A. The second letter was addressed to Veteran B. The letter contained Veteran B's full name, full address, last four digits of the social security number, date, time and location of medical appointments. The concerned Veteran A reported this information to the Compensation and Pension Office. The Acting Privacy Officer (PO) discussed this issue with the Compensation and Pension Management Staff. Update: 02/24/12:Veteran B will receive a letter or notification.
Outcome:
. The supervisor retrained the employee on handling sensitive information and mail outs. The employee re took the privacy training given by the Alternate Privacy Officer and both the supervisor and the privacy officer educated the employee on proper handling of information and patient sensitive data.
