Issue:

The Privacy Officer (PO) received a call from Regional Office Service manager this afternoon regarding offering credit monitoring letters to Veterans due to lost claim files. This is the first time the PO was notified of this incident. According to the Administrative Officer for Compensation and Pension, UPS is responsible for the lost records. There were approximately 87 Veteran claim files missing and all have been recovered except 27. Records were sent to the incorrect UPS location .Staff are still researching the actual number of recovered claim files for more clarification. Update: 12/08/11:Even though this is UPS's the VA will provide 87 Veterans a letter offering credit protection services.
