Issue:

A neurologist found an end of shift report at 5:30 pm on 10/3/11, unsecured in the VA cafeteria. The report was printed at 2:56 pm on 10/3/11, so the window it may have been unattended is from 3:00 pm-5:30 pm. The end of shift report is a listing of 34 full patient names, full SSNs, diagnoses, height, weight, age, gender, blood pressure, allergies, and ward. There is no indication that the information was compromised, however it is impossible to tell if someone copied any of the info. The Privacy Officer will follow up with Nurse Manager to identify the employee responsible. Update: 10/04/11:Thirty-four (34) Veterans will be sent letters offering credit protection services, due to full name, full SSN being exposed.

Outcome:

Nurse Supervisor reminded all staff to secure documents and not to take documents outside the office.
