Issue:
A Pharmacist placed a prescription bottle for Patient A into a bag for Patient B. The Pharmacist called Patient B to notify him of the error and and the prescription bottle was brought back the next day. The Pharmacist had transferred to the Syracuse VA recently from the Rome CBOC Pharmacy and says he was flustered. This was caused by human error alone. Update: 02/17/11:Patient A will receive a letter of notification since his medication information was exposed.
Outcome:
The Pharmacist was verbally counseled by the Outpatient Pharmacy Supervisor regarding proper procedure to confirm that patient's identity and prevent future errors.
