Issue:

A patient's daughter reported that mail received from VA was re-taped closed indicating to her that it had been previously opened. It was also addressed to Address Street instead of the correct Address Lake Drive. The mailed document was a copy of the patient's CT Scan with the patient's full name, full social security number, name and date of procedure, and diagnosis. Update: 06/21/11:The Patient will be sent a letter offering credit protection services, due to full name and full SSN being exposed.

Outcome:

This incident was discussed with the Radiology Supervisor and the Radiology Staff responsible for release of information. The Health Administration Services (HAS) Release of Information (ROI) Supervisor will be working closely with the Radiology Supervisor to assist in resolving any immediate or ongoing issues relating to release of patient information. The HAS/ROI Supervisor will review the entire Release of Information process that is completed by the Radiology Staff to ensure they are providing timely and quality responses for patient information requested.
