Issue:

Reported to the Privacy Officer by the Records Manager. A Nursing home patient with diagnosis of dementia was approached by stranger while in Extended Care Building. The stranger reported gave him rolled up dollar bills and asked him to hold while he took photograph of his wristband. Wristband included full name, full social security number and DOB as well as unit assigned to. Update: 06/19/12:The Privacy Officer (PO) interviewed the patient and patient's wife, who is his Power of Attorney (POA), regarding incident. Patient reported incident to grounds keeper and to nurse yesterday, who then reported to police service. There is currently an open police investigation. Matter was not reported to Privacy Officer in a timely manner.06/21/12:Patient will receive an offer for credit protection services. 07/16/12:Credit letter has been sent to Director's Office for signature. Still awaiting signature. The PO will scan and upload redacted copy when received.
