Issue:

A Veteran submitted a request to the Release of Information Office for a copy of his medical records to be mailed to his residence. When the box of medical records arrived at his residence, along with his medical records, the box also contained the medical records of two other Veterans. The Veteran returned the other copies and those are now in the possession of the Privacy Officer. The records contained full name, full SSN, and medical information. Update: 04/03/12:Both Veterans will be sent letters offering credit protection services.

Outcome:

PO has completed and mailed out CM Letters to both Veterans. The Chief, Business Office and the employee's Supervisor is in the process of taking appropriate action against the Release of Information Clerk. In addition, the Supervisor of the Release of Information will hold training with all ROI Clerks to emphasize the need to check each request to insure the records of other Veterans do not get intermingled and then mailed to the wrong Veteran.
