Issue:

A Release of Information (ROI) clerk mailed Patient B's information to Patient A. The Privacy Officer (PO) is in the process of contacting Patient B to see if he received Patient A's records. Patient B's records contained his name, address, full SSN, and protected health information (PHI). Update: 06/10/11:The Alternate Privacy Officer spoke to Patient B and he stated that he did get Patient A's records. When asked what he did with them he told her that he destroyed them by ripping off the part of the records that contained the patient's name, address and SSN and threw it in the toilet and flushed them. The rest of the document went into the compactor.Veteran A and Veteran B will receive a letter offering credit protection services.

Outcome:

All employees involve were issued a written counseling. They were educated on how to use the double check system that's in place.
