Issue:

The Canandaigua Privacy Officer (PO) failed to lock a file cabinet as instructed by the Acting Health Information Management (HIM) Manager. Subsequently, employees removed their own employee files from the file cabinet. The VISN PO handling the incident since the facility PO was involved in the incident. Update: 01/23/12:The VISN PO is awaiting completion of the Reports of Contact (ROC).01/24/12:The VISN PO received the ROCs from staff and Managers. Based on her review, it appears that employee records were only taken by the respective employee.01/26/12: It was subsequently reported that one of the employee files is missing. This was reported to the VA Police and to Human Resources (HR) for review and investigation.02/06/12:One employee will be sent a letter offering credit protection services due to full name, full SSN and DoB being exposed.

Outcome:

Records were moved to a secure location. Reported to local police for investigation.
