Issue:
A VA Physical Therapy clerk found six DD214s, a VBA payment remittance coupon, and a military retirement pay letter in among magazines located in the waiting area. Update: 08/06/23:The six DD214s contain the full SSSN. The military retirement pay letter also contains the full SSN. These 7 Veterans will receive a letter offering credit protection services. The VBS payment remittance coupon contains the name address and file number. This veteran will receive a notification letter.
Outcome:
We are uncertain as to how the DD214 documents appeared in PT area. The schedule was checked for VBA employees who may have had an appointment within the last few months. No current employee on the schedule. The good news, PT clerk found and reported this incident.
