Issue:
Veteran A received Veteran Bs Means Test Anniversary Letter from the Health Administration Services (HAS) Health Benefits Advisor, Central Registration Team sometime after 10/16/13. The full Social Security Number of the Veteran was included in the letter. Veteran A hand carried Veteran Bs letter to Health Information Management (HIM) Release of Information (ROI) Office and the letter is back in custody of the Agency on 11/06/13. Immediate action was taken to notify Chief HAS to cease sending letters with the Veterans full Social Security number . Chief HAS responded corrective action is underway and no further letters with the Social Security number included will be sent.
Outcome:
11/06/13: Veteran B will receive a letter offering credit protection services.
