Issue:

A UPS package containing two controlled substance prescriptions inside are reported as lost/stolen from UPS in the West Lafayette, IN shipping depot. This package was last reported as "attempted to sign" at the patients door but patient was unavailable for signature. The package was returned to the West Lafayette, IN UPS depot but has not been located for a second attempt for patient signature. UPS has sent their official notification today (2/28/11) that the package cannot be found. Update: 03/01/11:This is a UPS issue, but did include Date of Birth so the Patient will be offered credit protection services.

Outcome:

Credit Monitoring Letter mailed to Veteran. This was not a VA error.
