Issue:

There has been a medication error and privacy breach in pharmacy. On Friday 8/29/14, Veteran A received Veteran Bs medication and medication information sheet. Veteran A noticed the error and returned the medication and medication information sheet to the pharmacy on 09/02/14. The medication label contained: patient name, drug name, medication directions, and script number. The medication information sheet contained: patient name, drug name and drug information, refill slip, patient mailing address and phone number. The medication was dispensed incorrectly on 08/29/14 at 4:39 pm. A pharmacist dispensed the incorrect medication to the patient. The patient noticed the error once he arrived at home and returned it (medication and paperwork) to the pharmacy on the next business day which was 09/02/14.

Outcome:

09/04/14: The Incident Resolution Service Team has determined that Veteran B will be sent a HIPAA notification letter due to Protected Health Information (PHI) being disclosed.
