Issue:

A Community Based Outpatient Clinic (CBOC) contract employee reported that Veteran A received Veteran B's x-ray report in error. The x-ray report was stuck to Veteran A's lab result. Veteran A received Veteran B's name, address, full SSN and diagnosis in error. Update: 01/19/12:Veteran B will be sent a letter offering credit protection services.

Outcome:

The CBOC staff have been reeducated about better QA processes to check each document and verify the correct name before placing in an envelope.The CBOC clinic manager will procure Softwork (fingertip moistener) to assist staff when sorting documents.
