Veteran A received a copy of Veteran B's x-ray results in the mail. The document contained Veteran B's full name and last 4. Veteran A has returned the documentation to the medical center outpatient clinic and alerted the staff. Update: 05/25/12:Veteran B will be sent a letter offering credit protection services due to full name and full SSN being disclosed.
Outcome:
Proper procedure for patient notification reviewed with staff. Copy of minutes provided to the Privacy Officer.
